About the program
Healthier Together is a community-driven, multi-year, placed-based, funding approach developed by Palm Health Foundation. This strategic initiative convenes neighborhood leaders and residents, listens to their needs, and provides financial support and resources to help the neighborhood make positive, healthy changes.
